Some of them have a bolt in the bottom (think X-trail does but its dark and cold out) that you can do up to stop it rattling. However that becomes a pain in the butt when you want to remove it of course.

Switch $140. Yes anyone can do it. Remove the shroud around the steering column and take the switch off which is only a couple of screws as I remember and the plug.

Full kit for King Cab fog lamps is $525. Or get some lamps you are happy with and fit the Nissan switch (headlamp/indicator switch with fog lamp part on it) for $140.

Is your D40 a spain built one or a Thai built one? If you are not sure does your VIN number start with VS (Spain) or MN (Thailand)? If it is a King cab or single cab it is Thai. If it is an STX dual cab it is Spanish. If it is an RX dual cab it could be either. Should just need the replacement...
