You havn't mentioned changing the fuel filter, that is the first thing I would do. It has fixed my problems in the past.

Yes, fuel filter has been changed.

I'm thinking it could be an issue related to the cam shaft or crank angle sensor?

Having compression test done to rule out other factors,
