QD32 engine issue

Hey guys, been having an issue with my 1997 d22 navara and i cant seem to work out what it is. Engine starts and runs fine at idle but as the revs rise it misses occasionally at first and more often with more revs. As a result the fuel consumption is terrible and the power is down a bit too. When it misses it also blows out a puff of greyish/white smoke. It has 230000klm's too so its not that old either and has always been well looked after. Would love to know the solution so i can fix it but i cant seem to get a definate diagnosis. Any ideas or anyone had this issue?
 
i would start by checking for air in the fuel lines. fit a bit of clear fuel hose on the pump outlet and look for bubbles.
also check for any fuel restriction, eg dirty filter, clogged up tank pickup, blocked tank breather (air sucking in when you undo fuel cap).

after that its most likely injection pump itself.
